If you’re wondering why there is such a disparity in healthcare insurance costs between
similar states like Wisconsin and Minnesota, it’s because Republican Governor Scott Walker
rejected enhanced federal Medicaid dollars and Wisconsin Republicans decided not to
implement more robust rate reviews. Translation: Republicans rejected money to expand
Medicaid and Republicans choose not to oversee rate regulation (if this doesn’t ring a bell,
when Republicans discuss it, they call their refusal to check corporation’s “freedom” and
“free markets”).

These decisions may cost Wisconsinites $1,800 or more a year over what their Minnesota
counterparts pay for health insurance. Wisconsinites will pay between 79% and 99% higher
premiums before tax credits are applied, and the middle class will be hit the hardest. Some
cities will pay as much as 136% higher premiums.

Contrary to the mantra the Obama admin has been pushing since the site launched, it's not slow and clunky because it's so popular, it's because of bad code...mainly caused by a disconnect from the developers....We recently launched a new eCommerce web site at my company and it was slow for pretty much the same reasons...but, it got fixed quickly because we had far fewer developers...the feds DID not need to have this many hands in the pot..

More Than One Million Health Insurance Policies Wiped Out So Far Because Of Obamacare, 15 Million More On The Way…October 24th, 2013


Owners of individual health-insurance policies already have received more than 1 million cancellation notices, according to a compilation of news reports.

But the pain likely won’t end there.

Health-policy expert Bob Laszewski has concluded that about 16 million Americans will lose their current plans because of Obamacare.

Laszewski explained that about 19 million Americans are in the individual health insurance market.

“Because the Obama administration’s regulations on grandfathering existing plans were so stringent about 85 percent of those, 16 million, are not grandfathered and must comply with Obamacare at their next renewal,” he said.

The 16 million, he said, will receive letters from their carriers saying they are losing their current coverage and must re-enroll to avoid a break in coverage .

“Most of these will be seeing some pretty big rate increases, he said.

Blue Cross in New Jersey, alone, is dropping more than 800,000 customers due to Obamacare…

Where is American health care now and where is it headed? Toward less choice by you and more control by bureaucrats. Toward more control by others over your body and life. Toward totalitarianism. This occurred in Germany. National health care in Germany, 1918-1945, is reviewed in this article.

The logic of national health care has to lead to more control by bureaucrats because its goals and poor incentives cause such high costs that the government seeks ways to control costs and decides how to ration medical services. Its control invariably will grow. In the name of health care, the government will decide who lives and dies, who gets an operation and who doesn’t, and who must wait. It may get into or deepen its control of diet, exercise, eugenics, sterilization, euthanasia, vaccinations, abortions, organs, and disease prevention. The government will want to control what you eat and drink, your sanitation, and any activity that has a risk that you may be injured or even exposed to something that might require medical attention. Activities such as pregnancy, sports activities or driving a car will all be fair game. Required procedures will be on the agenda of control. Since life is filled with risks, government activists use health and program costs as a wedge to gain control of personal lives and choices.

Obama supporters will stop at nothing to clear their president of any economic wrongdoing. It is especially obvious when it comes to defending the Affordable Care Act, aka Obamacare. Whether they're claiming that the ACA hasn't noticeably affected full-time employment or using ratios that don't tell the whole story, their cheerleading efforts are astounding.

But they cannot hide the grim truth: Obama has done a terrible job for the economy. Forgetting his health care reform for a moment, let's look at employment since its last peak.

For the first time since the statistic has been kept, employment hasn't totally recovered. So despite overheating the money-printing press, increasing taxes, and spending billions upon billions to "save" the economy, employment still hasn't recovered after nearly 70 months.

"Those who cannot remember the past are condemned to repeat it", said Spanish author George Santayana, and this is exactly why the economy is still sluggish. Just like Hoover and Franklin Roosevelt, Obama has increased the debt and hindered recovery.

As if his general handling of the economy weren't catastrophic enough, Obama decided to implement the ACA in 2010. Its effects on the economy are well known, even though some decide to ignore them. They are so dramatic that even many influential Democrats want to delay the law's individual mandate one more year. Former supporters are rallying against it because they've realized it means more unemployment.

It also means more part-time employment, as can be shown by this graph taken from the St. Louis Fed.



Since Obamacare became "law of the land", part-time employment is on the rise. What the Employment Policy Institute was referring to when it claimed that the ACA didn't boost part-time employment was part-time employment due to slack work or business conditions. Part-time employment has indeed declined in terms of raw numbers. However, a real economist would look beyond this data and think of what can be unseen.
And that which is unseen includes getting a part-time job because nothing else is available or even for non economical reasons.

And if you thought this general portrait of the economy was depressing, wait until you see the numbers for millennials (people 18-24 years old, for the purpose of this article). Millennials occupy 17% of all part-time jobs, despite forming just under 10% of the population, and the trend does not seem to want to improve. It's especially bad for white and Latino men, while black and Asian women fare worse than their male counterparts. And for those looking for a full-time job, good luck. Millennials, especially males, still haven't recovered their full-time employment rate from before the recession. The employment dropoff even affects those with an education. College graduates between the ages of 20 and 24 have a 10.8% unemployment rate, compared to a rate of 7.3% for the population at large. The unemployment rate for millennials with a master's degree jumps up to a stunning 17.2%, something that may explain the steady decline in millennials' participation rate in the workforce.

So unless Obamacare is repealed altogether, don't except the employment situation, especially for millennials, to improve anytime soon. Since we are usually less experienced, employers won't hire us — many hold back — or will only do it part-time so they won't have to pay our health insurance.

What the Actuarial Values in the Affordable Care Act Mean

The Patient Protection and Affordable Care Act (PPACA) establishes four levels of coverage based on the concept of “actuarial value,” which represents the share of health care expenses the plan covers for a typical group of enrollees. As plans increase in actuarial value – bronze, silver, gold, and platinum – they would cover a greater share of enrollees’ medical expenses overall, though the details could vary across plans.

The levels of coverage provided for in the PPACA are central to the coverage people will get and how they will ultimately perceive the effects of the health reform law. But an actuarial value is not as intuitive for people as specific deductibles and other out-of-pocket costs. Additionally, estimates of deductibles and other coverage features were not released by the Congressional Budget Office during the legislative debate.

Because projections of deductibles are subject to variation in estimating techniques and databases used, to present the most likely range of out-of-pocket costs, the Foundation commissioned separate analyses from three different actuarial consulting firms to present a range of potential estimates.

The study is part of the new Kaiser Initiative on Health Reform and Private Insurance, which will inform federal and state policymakers as they implement the PPACA and examine the implications of changes in the private insurance market under the health reform law.

On February 24, 2012, the Department of Health and Human Services issued a bulletin on actuarial value and cost sharing reductions.

Beginning with plan years starting on or after January 1, 2014, the Affordable Care Act will require non-grandfathered individual and small group market plans offered on and off the Exchange to provide coverage at specific actuarial value categories.

How are actuarial value categories defined?

Categories are defined by the average share of total health spending on essential benefits paid for by the plan. The ACA identifies specific actuarial value categories as "metal levels" specified as bronze, silver, gold and platinum. Bronze plans have the least generous cost coverage, and platinum plans have the most generous cost coverage.

Coverage levels are as follows:

Bronze = 60 percent of the actuarial value with respect to essential benefits
Silver = 70 percent of the actuarial value with respect to essential benefits
Gold = 80 percent of the actuarial value with respect to essential benefits
Platinum = 90 percent of the actuarial value with respect to essential benefits
Are non-essential benefits included in actuarial value calculation?

Benefits and services not included in the definition of essential benefits will not be included in the actuarial value calculation. The ACA provides 10 statutory essential health benefit categories:

Ambulatory patient services
Emergency services
Hospitalization
Maternity and newborn care
Mental health and substance use disorder services, including behavioral health treatment
Prescription drugs
Rehabilitative and habilitative services and devices
Laboratory services
Preventive and wellness and chronic disease management
Pediatric service, including oral and vision care
Who is developing the actuarial value guidelines?

HHS is charged with developing guidelines to provide for a de minimis variation in the actuarial valuations used in determining the level of coverage of a plan to account for differences in actuarial estimates. HHS proposed allowing for an actuarial value de minimis variation of 2%. For example, a gold plan [80% actuarial value] could have an actuarial value of 78% to 82%.

States will have the option to use a national standard population, or to develop their own standard population based on state claims data. States would also have the option to modify the national standard population using demographic and other adjustors in accordance with sound actuarial practices.

Are HSA contributions included in the actuarial value calculation?

For group health plans, employer contributions to a health savings account will be included in the actuarial value determination. HSA contributions paid directly by the individual would not count towards actuarial value.

Employer contributions to a health reimbursement arrangement also impact actuarial value. Per IRS rules, employees cannot contribute to an HRA. no one can help you....this doesn't make your company feel 'responsible' for you....it removes the benefit from
their hands......

Is there a publicly available calculator?

HHS intends to develop a standard calculator that would be publicly available for issuers to use to determine the actuarial value of their products. The actuarial value calculator developed by HHS would allow for issuers to input cost-sharing factors in order to determine the actuarial value of a product.

What are the most important parameters for determining actuarial value?

According the HHS guidance, the factors that impact actuarial value the most include: deductible, co-insurance, maximum out-of-pocket, and, to a lesser extent, specific cost sharing on ER visits, inpatient admissions, pharmacy benefits, laboratory services, and diagnostic imaging.

Does actuarial value equal plan design?

Plans with the same actuarial value will rarely have the same plan design. In the context of the ACA, actuarial value is determined as if the plan were provided to a standard population, without regard to the population that actually receives benefits from the plan.

What is BCBSM doing?

By 2014, all small group and individual BCBSM plans sold on and off the Exchange must incorporate the actuarial value requirements.
